Use the FIFA 14 to FIFA 15 Hair Converter to convert the hair, use Face Master
to convert the head from 14 to 15, and then import the head with CM16

You also need to open the face file, export the image from the face and use it to create the mask.

The hair converter converts the hair file from 14 to 15, the 15 hair file also works in 16 so they don't need to be converted from 15 to 16.
