Happened with me as well with WE7..
I won a corner and lifted it in by giggsy, RVN tried to head the ball but his head hit the defenders head in the process
and there you go.. head injury for the defender

If you have no injuries, maybe the injury-option in the general settings is off?

i had a knee injury once where the player had bandage around his knee. Never had head injury before
