I have completed all challenges but one... the EFA cup. Did manager mode with different teams at different levels and I still haven't seen it yet.

Your manager mode teams were obviously all too good :)

The EFA is a secondary competition to the ECC (think real world UEFA Cup vs. Champions League) so if you always win the top division league title then it is the ECC that you will see. There are three ways of getting into the EFA - finish in a more lowly position in the top league in any given country (for the English Prem it is places five to seven, I believe), win a domestic cup competition whilst not finishing in a high enough league position to qualify for the ECC (that excludes special competitions like the Charity Shield in England or the various 'Super Cups' but does include both the league cup and FA cup), or qualify for the ECC and either get eliminated in the preliminary round (a difficult one, because you have to know which league positions only get a preliminary round spot) or finish third in an ECC group (which drops you into the EFA knock-out stages rather than the group stage).

My recommendation would be to play a career as a second tier league team (eg. the Championship in England, Serie B in Italy) and win a domestic cup competition. None of the lower leagues offer European places, so that should guarantee an EFA place.
